Service Description
This option is only for students who have already taken our CNA training course AND has a test date assigned by Prometric. You will have access to the lab room to practice the skills on your own or with other students who are also taking their refresher course at the same time. Your first 2 hours are FREE. You can pay an additional $25/hr in-person if you need more time. Please note: Additional time isn't guaranteed if the time frame doesn't allow. Please check your schedule before booking. If you miss your free practice time you will have to pay $25 to be rescheduled again. If you need additional practice time it's recommended to book all the time you need in advance to ensure there will be enough time and space. Please wear scrubs with crocs OR pants and a shirt with closed toed shoes. Please bring your study guide with you for reference and practice. It is important that you study the written material in your study guide as well as complete all of the practice questions in your study guide and on the websites listed on the last page in your study guide "Additional study material" - this will prepare you for the 60 multiple choice questions on your state exam. Please give us a call (904)651-1420 if you need to be rescheduled. If for any reason we have to reschedule your refresher course we will call you at least 24hrs before your scheduled time to do so. If you feel as though you need to retake the entire class again, you have the option to do so for $100- you must still have your original study guide for this option. Study guide reprints are $50. If you choose to retake the entire class again, please give us a call so we can assist you with rebooking. (904)651-1420

Please check your schedule before booking as All payments are final. If you need to be rescheduled, please call during One Week CNA Training's business hours to do so within 48 hours of your class start date or you will forfeit all funds. You may only be rescheduled once. If you have to be rescheduled more than once you will forfeit your deposit and have to register again online. If you are more than 20 minutes late to the first day of class your class will have to be rescheduled, and you will have to pay your deposit again. Language Proficiency Waiver and Acknowledgment By submitting this registration form, I acknowledge and agree to the following: • I understand that all instruction, materials, and communication for this program will be conducted in the language I selected at registration. • I confirm that I can read, write, and speak the selected language at or above a 6th-grade level. • I understand that this level of language proficiency is necessary to fully participate, comprehend course materials, successfully complete the program and pass the state exam • I understand that One Week CNA Training is not responsible for any misunderstandings, missed information, inability to complete the program or inability to pass the state exam due to my language skills. • I agree that refunds or special accommodations, including translators, phones, or recordings will not be granted for issues related to language comprehension or communication difficulties. • SPANISH ONLY- I understand the clinical portion of the state exam is in English and will need to read & understand
